See Mrs. Voehl, the school nurse to get a copy of your immunizations. This is a very important document that you must store in a very secure location. We advise keeping your immunization record together with your other important legal documents, like your car title. You WILL need these dates of your shots for vocational, college and military entrance, and for international travel.
