High School     Student Handbook     2008-2009    

Each athlete must have current 2008-2009 signed forms on file BEFORE any athletic participation begins.  Please click on the following links and print your form to be filled out.  Please be sure to fill out ALL FORMS COMPLETELY.  These forms may be turned into the High School athletic office any time after July 30, 2008.  Sports Physical forms must be updated every three (3) years.

E-mail addresses are very convenient for contact between parents and school.  We are asking all families with e-mail addresses to contact their child's school and leave an updated e-mail address.  If you frequent our "On-Line Parent Access" on the JCC Schools' website, you may enter your e-mail address there and submit it.  We will be able to download this information directly into your family account.  If you have any questions, please contact your child's school.
